I recognize that there are dangers inherent in the sport of basketball and it's training elements, and agree to assume all risks related to my/my child's participation. I release, waive, discharge and covenant not to sue Drive4Stardum, LLC., Donta Johnson, Owner, or any of the staff and employees from any claims, demands, or course of action whatsoever arising out of related to any loss, damage or injury, including death, that may be sustained by my child, or to any property belonging to my child, whether caused by negligence of the employees or otherwise, while participating in any given session, or while in, on, or upon the premises where the activity is being conducted. I hereby grant Drive4Stardum, LLC. the right to use photographs, video images and/or media of my child for advertising, publicity and other commercial purposes. Participants are expected to act in a manner that is not disruptive to staff or other participants. Any participant that acts in a manner that is unacceptable by the staff will be asked to leave the camp and will not receive a refund.

By signing below I acknowledge that i have read, understand and accept the above contractual agreement. I have read and willingly agree to the terms stated above. The contract is a binding contract and cannot be voiced for any reason.
