Drive 4 Stardum

Registration

Registering for a session, camp or fall ball requires the following:

* Waiver form
* Needs Assessment Form (If applicable)
* Application form
* Payments must be paid in full for the specific event(s) that the participant is registering for. Registering for an individual session(s) a contract will be developed and the terms of the agreement will be discussed between the participant and the President of Drive4Stardum.
* A consultation with parent/guadian will be conducted if the participant is under the age of 18 years of age. Topics to be covered could include but not limited to needs assessment, medical condition, date(s) and times, etc.
* Once Drive4Stardum has received your registration for an event an email confirmation will be made and the process begins. Please understand that all of the criteria above must be completed before the participant is able to beginning training with Drive4Stardum.
